Abstract
This video delves into the presentation, diagnosis, and surgical management of Terson syndrome in a 35-year-old man following a traumatic brain injury. The patient presented with vision loss in his right eye after experiencing a car accident and subsequent seizure, leading to hospitalization and right fronto-temporal-parietal craniectomy for subdural hemorrhage evacuation. Upon examination, vitreous and sub-hyaloid hemorrhages with shallow localized retinal detachment were diagnosed in the right eye. A 23-gauge pars plana vitrectomy (PPV) with internal limiting membrane (ILM) peeling was performed, resulting in significant visual improvement postoperatively. This case highlights the successful surgical management of ocular manifestations of intracranial hemorrhage in Terson syndrome, emphasizing the role of vitrectomy with ILM peeling in restoring visual function and underscoring the importance of interdisciplinary collaboration between ophthalmologists, neurosurgeons, and physicians.
