Abstract
A 60-year-old female, previously treated for cataract surgery, presented with diminished vision and elevated intraocular pressure 1.5 months post-operation. Examination revealed a 360-degree iridocorneal adhesion, leading to a diagnosis of aqueous misdirection syndrome. Despite initial management with atropine and prednisolone acetate eye drops, surgical intervention became necessary. The patient underwent multiple procedures, including a pars plana vitrectomy and repeated anterior chamber reformation attempts, using C3F8 and subsequently Healon. Persistent superior adhesions led to a third operation involving single pass four throw pupilloplasty, which successfully stabilized the anterior chamber structure postoperatively.
