Abstract
A canthus based peritomy, a minimal heat cautery and a 4.5 mm sclero-corneal / limbo-corneal tunnel with Blumenthal radial cuts and side-pockets is performed.
Minimal procedure in the bag and less of repeated entry of the instruments into the AC, is the hallmark of this surgery.
It induces the minimum SIA and so is performed in anastigmatic cases or cases having against the rule astigmatism.
Contrary to popular belief, a temporal approach is not at all unsightly in the post op. period and gives us good replicable results.
Key points:
1. Closed chamber rhexis.
2. Hydro-dissection and flip or cart-wheel out the nucleus.
3. Build up visco pressure in AC.
4. No other intra cameral maneuvers in AC for lens extraction. Just tap the lower lip and the nucleus glides out.
