Abstract

CASE REPORT
PURPOSE To report a case of orbital apex syndrome secondary to mucor mycosis.
METHOD A 50 year old female k/c/o DM and HTN came with the C/O DOV in LE with dropping of LE upper lid and left sided headache.She was diagnosed with fungal sinusitis prior.On examination LE had negative perception of light, upward deviation ,ptosis loss of corneal sensations with RAPD and restriction of ocular movements in all gazes.Fundus was normal.Right eye was within normal limits.
RESULT: She was treated with amphotericin b for 5 days elsewhere and later presented to us.CT and MRI showed sphenoidal osteomyelitis and cavernous sinus thrombosis and she underwent FESS followed by 4 doses of amphotericin B and Tab Posaconazole 100mg.Post treatment she improved symptomatically but vision loss and restriction of eye movements persisted.
CONCLUSION The cause of the orbital apex syndrome determine the treatment.Infective cause as in our case require early treatment to save the life of the patient .
