Abstract
Here we present a case of soft posterior subcapsular cataract in a diabetic being taken up for 3.5mm Manual SICS.
An oval ccc is fashioned.
An oval rhexis prevents entrapment of the fluid between the PC and soft cortex.
A soft cataract would not engage into the dialer .
A gentle hydro-dissection is carried out with 26-gauge cannula and the nucleus is tapped on the opposite side to release the trapped fluid.
In soft cataracts, we shift to visco cannula at this step. Visco does not let the A C collapse and gives a cushioning effect on the P C.
Delivery of the nucleus is with visco expression.
Manual SICS gives a more stable chamber.
Visco aids the prolapse, protects the PC and prevents sudden decompression of the chamber.
Cortical wash should be very meticulous.
A hydrophobic foldable IOL is implanted into the bag with IOL holding forceps, without an injector.
Thus soft posterior subcapsular cataract should not be a trouble.
