Abstract
Surgical management of intumescent mature cataract has always been a challenge in terms of achieving an intact, well sized continuous curvilinear capsulorrhexis owing to poor visibility due to absence of red reflex , transient visual disturbances caused by rapid egress of liquified cortical matter & high intralenticular pressure posing a greater risk of rapid uncontrolled radial anterior capsular tear extending to periphery during the cataract surgery.
Meticulous pre-operative measures and numerous surgical modifications have been attempted ever so often to overcome these challenges. The widely practiced strategy includes decompression techniques.
This video illustrates one such approach known as 2 staged or mini capsulorrhexis technique as well as various pre operative, intraoperative and peri-operative measures taken alongwith it to minimise complications during surgical management of intumescent mature cataracts.
