Abstract
AIM:Study of rare case of OPN in young patient of TB meningitis(TBM) on ATT(ANTI TUBERCULAR THERAPY) with no other fundus findings of ocular TB.
METHODS:23 yr old female k/c/o TBM on ATT referred from pulmonary dept with c/o gradual onset headache,eye pain,blurring of vision both eye(BE).
INVESTIGATIONS:BCVA RE 6/12(P) LE 6/18(P).
IO: peripapillary ring shape elevation LE>RE.
Pupil RE RRRL LE RAPD Gr1. IOP BE 14mmHg NCT.
MRI: Mild prominent B/L optic nerve sheath.
30 days later, BCVA RE 6/6 with -0.50D CYL @90 LE 6/9 with -0.25DCYL @90 binocularly 6/6(P).
Pupil RE RRRL LE Gr1 RAPD Colour Vision(CV) no deficiency BE
IO: peripapillary ring shape elevation LE>RE , temporal pallor of optic disc LE.
SYSTEMIC: PCOS & ATT induced liver injury.
F/U 4 month later VA with glass 6/6 unaided VA 6/6(P) BE. No CV deficiency. Fundus normal. C/o headache.
Treatment: ATT(modified regime). Ethambutol stopped. Tb methylcobalamine 1500mg OD. Tb dexamethasone(4mg) in tapering dose.
