Abstract

This case report aims to highlight an atypical presentation of the primary ductal adenocarcinoma of the lacrimal gland. A 48-year-old male presented with outward protrusion of the right eye and swelling around the right forehead for four months, not associated with any visual complaints. On examination of the right eye, there was abaxial proptosis with downward and outward dystopia, limited extraocular movements, diffuse congestion with prominent episcleral vessels, lagophthalmos of three mm with BCVA of 6/6 and few choroidal folds in the fundus. A diffuse forehead swelling with temporal fullness was present, continuous with upper lid fullness. The left eye was unremarkable. A contrast-enhanced CT revealed a mass occupying the right orbit in the extraconal region with intracranial spread and significant bony destruction of the frontal bone and lateral orbit. Histopathology examination revealed primary ductal adenocarcinoma of the lacrimal gland.
