Abstract

Purpose
To share a rare case of synchysis scintillans in anterior chamber
Methods and Results
A young man presented with pain & redness in his LE for a month. Examination revealed a vision of 6/9 partial in RE and esotropia in LE with NPL, with both the inward deviation and NPL present from childhood. He denied any trauma.
Biomicroscopically RE was normal, whereas LE showed conjunctival congestion, band shaped keratopathy inferiorly and multiple highly mobile refractile particles in the AC continuing with similar findings in the anterior vitreous, through the mid-dilated and fixed pupil, while the lens was subluxated inferiorly. A diagnosis of synchesis scintillans was made.
The IOP of RE was 17 mm Hg, while LE had hypotony. Fundus of the RE revealed a large chorio-retinal coloboma in the supero-nasal quadrant; the fundus of the LE could not be visualised.
Conclusion
Synchysis scintillans appears in diseased eyes and lens subluxation may cause crystals to migrate in AC.
