Abstract
A 72-year-old male with pseudoexfoliation and a bulky nucleus underwent cataract surgery. This surgical video provides a comprehensive overview of an advanced cognitive planning strategy tailored to address complex scenarios in cataract surgery, specifically those involving a bulky nucleus and zonular laxity.
The three-step protocol employed in the video combines the precision of the Mackool capsular hook for stabilizing the capsular bag, the symmetric effect of a standard capsular tension ring to counteract zonular laxity, and the robust structural support provided by a standard capsular tension segment. A sclerotomy was performed, followed by a misdirected needle entry that inadvertently touched the posterior capsule and was effectively managed.
