Abstract
In a 80 year old lady with deep set eyes, grade 3/4 cataract, moderately dilating pupil, with pooling of fluid, difficult visibility and focus, continuous shallowing of anterior chamber due to posterior synechiae , the IOL was used as a scaffold to protect the posterior capsule and emulsification of the rest of the nuclei and residual lens matter was carried out with utmost patience. The visual outcome was successful although the striae keratopathy subsided within 2 weeks. Using the IOL as a scaffold was the best possible option in this case and should be often used in difficult cases of anterior chamber shallowing during surgery.
