Abstract
GATT is an ab interno conjunctival sparing minimally invasive glaucoma surgery (MIGS) that works by reducing resistance at the level of the trabecular meshwork[TM] and the inner wall of Schlemm’s canal[SC].Advantages of GATT over trabeculectomy are,step-wise approach with the possibility of filtering procedures later in the course of the disease, no bleb-related complications, lower rates of hypotony etc.Two clear corneal paracenteses are created.Cohesive viscoelastic device
is injected into the anterior chamber[AC].After creation of temporal corneal wound, the patient’s head and the microscope are tilted away from the surgeon. The angle is then visualized using gonioprism, and a 2‑3 clock hour goniotomy is created using MVR blade. Once SC had been visualized, the distal end of the Prolene suture is guided and advanced continuously through the canal.The suture is then internalized into the AC, cleaving the inner wall of Schlemm’s canal and the TM.AC wash is done at the end.
