Abstract
The patient presented to the eye outpatient department with a history of diminution of vision in both eyes. Cataract was workup by slit lamp examination following pupil dilation, 6 clock hour subluxation was noted nasally. Various blood investigations were conducted, revealing NR viral markers,a homocysteine level of 14.40 for homocystinuria was normal and normal RBS at 94. Additionally,B-scan revealed anechoic vitreous with visible optic nerve head. Systemic examination for Marfan syndrome was normal. The patient underwent cataract surgery via phacoemulsification with trans conjunctival tunnel-assisted phacoemulsification + three piece lens under local anaesthesia. Capsular support with CTR was tried but due to greater subluxation anterior capsule around rehxis margin given away. So hook was used and surgery was performed without any support device by successfully placing the lens in the capsular bag. On postoperative day 1, the lens remained in place without any signs of subluxation.
