Abstract
A 12year old girl presented to us with chief complain of right eye diminision of vision.Parents were concerned about right eye looking abnormal occasionally when looking at left gaze. On extraocular examination, right eye was having mild adduction limitation with right eye widening of palpebral fissure on adduction and narrowing of palpebral fissure with globe retraction on abduction. A diagnosis of congenital cranial dysinnervation disorder probably inverse duanne's retraction syndrome due to congenital etiology was diagnosed. There are only 4 cases of it reported till now. This is how a small routine eye examination can lead us to find rare entity.
