Abstract
Intumescent cataract is a condition characterized by the significant swelling of the lens due to the osmotic imbalance caused by the accumulation of fluid. The influx leads to swelling leading to a milky white appearance with increased intralenticular pressure, which can cause the anterior capsule to bulge forward
and prone to radial tears, making surgical intervention challenging and increasing the risk of complications. This technique combines soft-shell technique and gentle massaging of the anterior capsule to break the fluid pockets and creating intralenticular communication channels between those pockets. This allows for the fluid to escape from the central puncture created by rhexis forceps while the peripheral capsule is prevented from rupture by the visco-cohesive agents. An adequate rhexis can thus be performed without any additional measure.
