Abstract
Microspherophakia is a developmental lens anomaly with increased antero-posterior and reduced equatorial diameter. It presents with refractive myopia, shallow anterior chamber, and angle closure glaucoma. Glaucoma is the most common vision threatening complication and mostly requires surgical management that includes trabeculectomy, lensectomy or drainage implant. A staged or combined procedure can be performed. The purpose of this video is to illustrate quadruple procedure (non-valved aqueous drainage implant, parsplana lensectomy with vitrectomy and scleral fixated intraocular lens) in a microspherophakic patient with gross lens subluxation, advanced secondary angle closure glaucoma at risk of progression and losing central vision. The authors also emphasize the advantages of combined quadruple procedure over staged procedure. It eliminates the need for multiple procedures, risk of hypotony and bleb related complications.
