Abstract
Along with pupillary reaction USG in mature cataract should be carried out prior to surgery to rule out any posterior segment pathology . This will help us in prognosticating visual outcome and avert dreadful complication during surgery .We present a 60/F referred to us for evisceration of RE with the diagnosis of expulsive haemorrhage .The patient was operated for breast cancer 2YR back .The patient had undergone surgery for mature cataract in RE at local district hospital two days prior to the presentation .She presented with severe eye pain and headache for 2 days . O/E in RE had PL – , there was gaping in scleral tunnel with iris prolapsing & 3-4 suture at the scleral wound . Cornea was oedematous and AC hyphaema .IOP raised . The patient underwent evisceration of RE .During surgery there was profuse bleeding and uveal tissue was thick ,firm and adherent to sclera . The tissue was sent for histopathology suggestive of moderately differentiated adenocarcinoma.
