Abstract
Background: Carotid-cavernous fistulas are an uncommon disease characterized by abnormal communication between carotid artery and veins located in the cavernous sinus. We report a case of direct carotid-cavernous fistula (CCF) that occurred after a motorbike accident, an uncommon disease after trauma.
Case presentation: A 57- year-old man complained of left eyelid swelling, eye redness, decrease in visual acuity and occasional headache 6 days after motorbike accident. On examination there was pulsating exophthalmos and limitation of extraocular movements of right eye. CT Angiography scans showed dilated right superior ophthalmic vein with enhancement of right cavernous sinus suggestive of Carotid Cavernous Fistula (CCF). We decided to treat this case with endovascular approach after considering several therapeutic options.
Key words: Carotid cavernous fistulas, pulsating exophthalmos, orbital blow, endovascular approach, Barrow´s classification.
