Abstract
Pachychoroid disease is characterised by presence of Pachy vessels, i.e., dilated Haller vessels with attenuation of choriocapillaries. The term itself means Thickened Choroid which can be detected through Enhanced depth imaging. This disease is now been investigated and is rather a spectrum of disorder which undergoes through various phenotypes. The disease lies in the choroid, begins with exudation and then follows neovascularisation. The management lies in targeting the choroid. Hence the rationale regarding use of intravitreal injection of brolucizumab is promising. Brolucizumab is the antiVEGF with the highest molar mass owing to the smallest size of the molecule among the other agents that are being used for the management of retinal exudation and neovascularization. Its smaller size accounts for better penetration through retina and choroid and this property of brolucizumab is hypothesised to produce better therapeutic effect against the disease.
