Abstract
Purpose: To study cataract surgery outcomes in severe dry eye in rheumatoid arthritis(RA) patients with topical cyclosporin(0.09%)
Case1:RA patient with schirmers 0mm,cataract.Topical Cyclosporine(0.09%), systemic anti rheumatoid medications, oral steroids started 1week prior ,underwent cataract surgery(phaco) was on steroids 3weeks postop with Visual 6/6
Case2: RA patient schirmers3mm underwent cataract surgery with cyclosporine(0.09%) has improved schirmers10mm had post op vision 6/6 clear cornea.
Case3: RA patient with corneal melt 1month post cataract surgery treated with bandage contact lens+glue,after 1 month corneal thinning reduced and re-epithelized.with steroid cover,cyclosporine drops cataract surgery in other eye done with VA 6/6 clear cornea
Conclusion:Dry eye stabilization with topical cyclosporine(0.09%) prior to surgery has good visual outcomes without corneal melt.Team management with immunologists, systemic disease control prior to cataract surgery essential in RA.
