Abstract
Infective keratitis(like fungal, bacterial and acanthamoeba keratitis) is a common vision threatening disease of cornea. Untreated corneal ulcers, large non healing corneal ulcers or those which are not responding to medical treatment results in corneal melting, endophthalmitis, panophthalmitis and perforation causing severe ocular morbidity, even loss of globe. Therapeutic penetrating keratoplasty(TPK) is the only way to preserve the eyeball integrity in those which are severe, progressive not responding to medical management; corneal melts and impending corneal perforations. TPK thus saves the eye structurally and functionally. The main aim is to maintain the integrity of eyeball, remove the infective focus to prevent intraocular spread and scleral extension, visual rehabilitation is secondary. TPK can result in high therapeutic, anatomical and functional success. Thus demonstrating the surgical technic of therapeutic penetrating keratoplasty.
