Abstract
Many methods have been adopted to manage uveitis eyes with chronic hypotony. Traditionally removal of pars plana membranes, use of higher viscosity silicone oil and oil retaining sutures were tried to avoid anterior migration of silicone oil, thereby preventing corneal decompensation. The case in discussion presented with chronic uveitis with complicated cataract. UBM imaging revealed signs of hypotony, including complete blunting of ciliary processes and hypodense ciliary body.
Patient underwent vitrectomy, pars plana membrane removal, ciliary process freeing and oil retaining sutures before filling silicone oil.
The designs of oil retaining suture depends on complexity of case, wherein more sutures are desired in severe hypotonic eyes. Double-PLUS sutures in the present case prevented oil from anterior migration and emphasizes its utility in such complex cases.
The surgical video demonstrates techniques of ciliary process freeing and method of suture designing in such eyes.
