Abstract
A 15 year old boy presented with FCCF vision in his right eye following RD surgery(post traumatic) elsewhere.O/E anterior segment was WNL. Fundus examination revealed taut inferior retina with macula partially off under silicone oil complicated with a very large FTMH with Base diameter of 3827 microns and a sub retinal band(SRB) traversing the macula underneath the FTMH.After oil removal, posterior pole was stabilised under PFCL and retina was stained by BBG dye.ILM peeling was done under PFCL and multilayered flap was positioned over the large hole.SRB was pulled out through the large hole and ERM was removed to settle the retinal puckering near the inferior arcade. Fluid air exchange was done and retina was settled followed by endolaser and silicone oil injection. Postop BCVA was 20/200 with retina settled under oil and closed large FTMH.To conclude, most common cause of recurrent RD is PVR changes and you have to get rid of both epi&sub retinal membranes/bands for better outcome.
