Abstract
A 25-year-old boy presented with Total Retinal detachment with choroidal detachment. It was a 180-degree Giant Retinal tear. The nasal retina was folded over the temporal retina like a book. The case was taken up for Surgery. The choroidals were drained first, then 25 G PPV was carried out. The nasal retina was stroke-free from the temporal retina with the help of a silicone soft-tip cannula. Then, PFCL was injected between the two flaps, and the retina was settled under PFCL. Then, PPV was completed. Endolaser done. PFCL Silicone Oil exchange was done. Post-op, the retina was settled, and vision improved to 4/60 in the operated eye
