Abstract
White cataracts are characteristically swollen as they are full of cortical material enveloping a hard nucleus. They have weak zonular fibers and fragile capsules. Intumescent cataracts are a type of mature cortical cataract and follow the same pathophysiological progression. White cataracts present a high risk for complications even in hands of experienced surgeons. One of the greatest challenges in managing a white cataract is the creation of a continuous circular capsulorrhexis (CCC). Studies have shown that capsulorhexis that was not totally successful in the management of white cataract may be observed in 3.85% to 28.3% of cases with an increased incidence of posterior capsular rupture and vitreous loss. This video showcases technique of management of a white intumescent cataract employing 2-stage capsulorrhexis and cortical debulking using bimanual irrigation & aspiration prior to the phacoemulsification of the nucleus and subsequent successful IOL implantation.
